Welcome Referring Providers

We accept referrals for children, adolescents, and adults seeking:

• Individual therapy for anxiety, trauma, burnout, identity exploration, and life transitions
   
• Relationship and family therapy, including couples, partners, and family systems
   
• Autism and ADHD evaluations using neurodiversity-affirming approaches
   
• Group therapy and structured programs, as available
   

We welcome referrals for clients who may benefit from a thoughtful, flexible, and affirming therapeutic environment.

 Creekside Therapeutics offers comprehensive evaluations for autism and ADHD across the lifespan.

Evaluations are:

• Neurodiversity-affirming and strengths-informed
   
• Appropriate for diagnostic clarification, self-understanding, and support planning
   
• Available for children, adolescents, and adults
   
• If you’re unsure whether an evaluation referral is appropriate, we’re happy to consult.

Our practice includes licensed clinicians and residents-in-counseling. Working with a resident may allow for more immediate availability, while care remains closely supervised and aligned with our clinical standards.

We strive to match clients based on clinical needs, preferences, and current availability.

 To help us process referrals efficiently, please include the following information when available:

• Client name and age
   
• Presenting concerns or referral question
   
• Service requested (therapy, evaluation, group, etc.)
   
• Relevant diagnoses (if applicable)
   
• Insurance information or self-pay preference
   
• Client contact information
   
• Any timing considerations or urgency
   
• Your name and contact information
   

Incomplete referrals are welcome—we’re happy to follow up as needed.

1. We review the referral and determine fit and availability
    
2. The client is contacted to discuss next steps
    
3. If appropriate, services are scheduled and care is coordinated
